Privacy Collection Notice

NSW Health is collecting your personal information for the purpose of administering health promotion training and programs, such as the Munch & Move program. The details you provided will be securely on the Munch & Move Learning Management System website, owned and operated by the NSW Ministry of Health and in our Population Health Information Management System (PHIMS), owned and operated by the NSW Ministry of Health and our digital partner, eHealth NSW.

Personal information is defined as any information or opinion about an individual whose identity is apparent or can reasonably be ascertained. Any personal information supplied by you will be managed in accordance with the Privacy and Personal Information Protection Act 1988 (PPIP ACT). While you are not required by law to supply  your personal information, your personal information is required to provide you with health promotion training and programs.

 

What information do we collect?

This Privacy Collection Notice applies only to the personal information you provided in the registration form, this includes your name, role or position, service email address, preferred email address and service phone number.

Your personal information is used to register you in the Munch & Move eLearning training and may be used to communicate with you about the training, the Munch & Move program and to help us manage and evaluate the program and training.

How we use your information

The information you provided will only be used by NSW Health employees or approved contractors to enable us to contact you regarding the administration, implementation, performance monitoring and evaluation of our health promotion programs.

Who can access the information?

Only authorised users, who are NSW Health employees or their approved contractors, have access to view your personal information.

NSW Health will not disclose your personal information to anybody else unless required or authorised by law, such as by court order.

How we protect your information

NSW Health maintains robust security measures to protect personal information stored in our systems from unauthorised access, disclosure, or misuse, in accordance with the Privacy and Personal Information Act 1998.

Personal information collected and stored in NSW Health systems is retained in accordance with the State Records Act 1998.  Information no longer required for administrative or reference use will be securely disposed of to prevent unauthorised access or disclosure.
